The film follows Al Simmons (played by Michael Jai White), a highly skilled mercenary who is betrayed and killed by his corrupt boss, Jason Wynn (played by Martin Sheen). In death, Simmons makes a deal with Malebolgia, the ruler of the Hell, to return to Earth as a Hellspawn in exchange for his soul.
