After his fiancée Ruby dies from a heroin overdose, former addict Shelby John goes on a quest for revenge against the drug cartel responsible. He teams up with Sheriff Church to take down the cartel’s leader, Johnny Black. In the climactic final confrontation, Johnny holds Ruby’s young nephew hostage and prepares to kill him. Shelby begs Johnny to spare the boy, offering himself up instead. As Johnny prepares to execute Shelby, Sheriff Church shoots Johnny in the back, killing him. With the cartel dismantled and Johnny dead, Shelby has achieved his vengeance. But the violent acts have taken a toll on his soul. In the final scene, Shelby brings Ruby’s body to a river to fulfill her wish of being baptized. As her body sinks into the water, Shelby breaks down in tears over all he has lost. The ending suggests Shelby may find redemption through his pain and begin to heal. But it remains ambiguous whether he will fully escape the cycle of violence. Sheriff Church worries that Shelby has crossed a line and may not recover from his revenge quest.
