The film begins with college students Trish (played by Gina Philips) and her younger brother Darry (played by Justin Long) driving home through rural Florida for spring break. Along the way, they encounter a sinister-looking figure in a rusty old truck that appears to be dumping something down a drainpipe in the middle of nowhere.
Curious, Trish and Darry decide to investigate and discover a hidden cellar filled with human corpses that have been mutilated and sewn together. Horrified, they flee the scene and try to report the discovery to the authorities, but their efforts are met with skepticism.
As they continue their journey, Trish and Darry become increasingly paranoid and fearful, realizing that they are being pursued by the mysterious figure in the rusty truck.
