The story revolves around Miranda Grey, a respected psychiatrist working at a mental institution called Woodward Penitentiary. Miranda is married to her colleague Douglas (played by Charles S. Dutton), who is also the chief administrator of the institution. One stormy night, Miranda encounters a mysterious young woman named Chloe (played by Penélope Cruz), who claims to be a victim of sexual abuse.

Shortly after her encounter with Chloe, Miranda experiences a series of disturbing and inexplicable events. She begins to have hallucinations and nightmares involving a ghostly figure with a burned face. Things take a dark turn when Miranda wakes up to find herself locked in a cell in Woodward Penitentiary, accused of brutally murdering her husband.

As Miranda struggles to understand what is happening to her, she crosses paths with her former colleague Pete Graham (played by Robert Downey Jr.), who is now assigned to evaluate her mental state. Pete is skeptical of Miranda’s claims of innocence but agrees to help her uncover the truth behind her husband’s death and her own inexplicable experiences.

As Miranda delves deeper into the mystery, she uncovers shocking revelations about Woodward Penitentiary and its dark past. She learns of a former patient named Rachel Parsons, who was a victim of sexual abuse and died under mysterious circumstances. Miranda begins to suspect that Rachel’s spirit is haunting her and seeking vengeance for the injustices she suffered.

In the climactic final act, Miranda confronts the truth about her own involvement in Rachel’s death and the role that Douglas played in covering up the abuse at Woodward Penitentiary. With the help of Pete and Chloe, Miranda faces the ghostly presence haunting her and fights to reclaim her sanity and innocence.

The film’s ending reveals that Miranda was indeed innocent of her husband’s murder and that her experiences were the result of Rachel’s vengeful spirit seeking justice. With Rachel’s spirit put to rest, Miranda is freed from her nightmarish ordeal and able to move on with her life, though haunted by the memories of the horrors she endured.
