The film centers around Jack Mosley (played by Bruce Willis), an aging and disillusioned NYPD detective who is assigned to escort a witness, Eddie Bunker (played by Mos Def), to testify before a grand jury. The journey is supposed to be a routine task, involving transporting Eddie from the police station to the courthouse, which is 16 blocks away.

However, as Jack and Eddie begin their journey, they quickly find themselves targeted by corrupt police officers who are determined to prevent Eddie from testifying.
