The film is set in the fictional Lake Victoria, a popular tourist destination where thousands of college students gather for Spring Break festivities. Amidst the partying and revelry, a tremor caused by an underwater earthquake releases prehistoric piranhas from their long-dormant state in an underground lake.
As the piranhas swarm to the surface, they begin attacking and devouring anyone in their path, turning the tranquil lake into a bloodbath. Among the victims are several partygoers on a boat captained by Derrick Jones (Jerry O’Connell), a sleazy pornographer shooting an adult film starring adult film actress Danni (Kelly Brook).
Meanwhile, Sheriff Julie Forester (Elisabeth Shue) and her deputy, Fallon (Ving Rhames), attempt to evacuate the lake and rescue as many people as possible. They are joined by Seismologist Dr. Adam Scott (Adam Scott), who discovers the cause of the tremors and alerts the authorities to the imminent danger.
As chaos erupts on the lake, a group of teenagers led by Jake (Steven R. McQueen) and his crush, Kelly (Jessica Szohr), find themselves trapped on a sinking boat surrounded by ravenous piranhas. With time running out and the piranhas closing in, Jake and his friends must fight for survival against the deadly creatures.
